Community Health Worker Women's Health - Grant-funded

JOB SUMMARY

Provides outreach, resource connection, and advocacy for pregnant and postpartum patients receiving obstetric care at Progressive Community Health Centers. Serves in a supportive role as part of an interdisciplinary health care team to maximize patient health outcomes. Provides resources for patients through activities such as outreach, community education, social support, and advocacy. Must have a strong interest in women's health and be comfortable with addressing sensitive topics related to pregnancy and postpartum care.


* This position is grant-funded for a minimum of one (1) year with the possibility of becoming a permanent role and/or full-time based on operational needs.


Responsibilities

  1. Provide care navigation and peer support services to pregnant and newly parenting patients with complex medical and psychosocial issues. Navigation activities include but are not limited to: scheduling follow-up visits, providing reminders, encouraging self-management care, and addressing issues regarding barriers to obstetric or postpartum care.
  2. Support overall patient well-being by using strong observation and communication skills to coordinate care between the patient, caregiver(s), and community resources.
  3. Deliver health information using culturally appropriate terms and concepts.
  4. Provide patient education in a manner that reinforces prenatal and postnatal care guidelines. This may include one-on-one or facilitated group interaction.
  5. Collaborate with other departments (behavioral health, pediatrics, dental and primary care) to connect patients to additional services as part of their health care plan.
  6. Support grant-related and other quality health improvement projects such as health screening and value-based care initiatives.
  7. Provide referral information and support to patients regarding access to social services and community resources, as needed.
  8. Participate in data collection, record-keeping, and data entry for assigned programs. This includes the use of various computer software to accurately interpret patient registry information including an EMR.
  9. Assist with other job-related duties as may be assigned.

Minimum Qualifications


Required: High school graduate, or Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED).


Preferred: Advanced training beyond high school that includes the completion of an accredited or approved Community Health Worker program.



EXPERIENCE:


Experience in a community-based agency, community health organization or related field.


KNOWLEDGE, SKILLS, AND ABILITIES:


Required: Excellent written and verbal communication skills and critical thinking skills.

Comfortable working with culturally diverse populations.

Ability to handle sensitive and confidential information according to policies.

Basic computer and phone skills, including knowledge and experience with data entry, Microsoft Office, and Internet web browsing. Ability to gain knowledge of local community agencies.
